Please note that several of the above plans require a referral from the primary care physician in order to receive insurance benefits. It is the patients responsibility to assure that a proper referral has been issued and is available to our staff at the time of the visit so that we may file on your behalf.

In order to reduce billing expenses, payment or copayment is requested at the time of service unless other arrangements have been made with our office in advance.

For your convenience we accept cash, personal checks, Visa and Mastercard.

Our office is capable of electronic claims submission. At your request we will submit your claim electronically via our computer at no additional cost if your insurance carrier allows it. This usually results in faster processing and reimbursement. Alternatively, we will provide you with a completed insurance form (HFCA 1500) for you to submit.
